When we look at women who’ve come to the office to talk about a mommy makeover, we can divide people into different body types. We know there’s the ectomorph, the endomorph, the mesomorphs. So everyone is going to have a little bit different body type, and what that means is that we’re going to address different things for different people. So everyone’s going to have a little bit of a different body, everyone’s going to have a little different shape, and there’s going to be things that each person wants to improve that may be different from another.

But classically, we’re going to be looking at doing something for their breasts to lift them, and doing augmentation, mastopexy procedure. We’re typically going to be doing something to the tummy to tighten those loose muscles, and do a tummy tuck, and there’s different kinds of tummy tucks, and it’s not a one-size-fits-all operation. And I can offer each individual person, the tummy tuck that fits their body type and their look, because it’s not a one-size-fits-all procedure. You have to tailor to each body type, knowing that you want to keep the scar low, knowing that you want to keep a nice looking belly button. So we follow those goals to give everyone the look they’re looking for.

In terms of liposuction, we’re usually doing some liposuction, sometimes on the waist, sometimes the inner thigh, sometimes on the saddlebag lateral thighs, sometimes on the inner thighs or even on the back, what we call the lumbar or the low back area, on the backside. Sometimes that’s part of it too. So it really just depends upon what areas that you need to be addressed.

People are often interested in losing weight and having a tummy tuck. And they come into the office and some people know that they need to lose some more weight, some people are uncertain how much weight they need to lose. And so, this is going to be just a brief overview of weight loss prior to tummy tuck. We know that a tummy tuck removes extra skin. We know that it tightens the muscle and we know it removes extra fat, but we really want people to be within about 10 pounds of their goal weight. Everyone has a different goal weight, but the key really is, is there as fat that can be stored inside the body cavity called intra-abdominal fat. And I can’t remove that. It’s not accessible to liposuction, it’s not accessible for surgical removal. And so patients need to understand that they really need to do their part first and get close to their goal weight because if you are not close to your goal weight, I can’t achieve the best result for you.

And I will tell someone that, I’ll say, “Yeah, I think we’re close, but if you’re going to lose another 10 pounds or 15 pounds, you probably should do that first. Because what that will allow is for me to take out some extra skin, tighten your muscles a little more, and ultimately your result will be better.” If I said, “Hey, go ahead and do it now.” And then you lost 15 to 20 pounds, what’s going to happen? You’re going to now have some loose skin and your muscles aren’t going to be as tight and ultimately your result won’t be as nice.

The day of your tummy tuck is a very big important day. And there’s a lot of things we have to manage before you have your tummy tuck, and on the day of surgery. The day or two before your tummy tuck, we’ll have you maybe on a low residue diet trying to clean out your intestinal tract because constipation is very common after a tummy tuck.

So the day of surgery you’ll be at the surgery center about an hour before we start, you’ll get all prepared by the nurses and anesthesiologists. I will stand you up and draw a few lines on your abdominal wall. Typically where we’re going to do the surgery. I’ll oftentimes put some circles on your hips or waist or other areas that we may be doing a little bit of liposuction. And so we’ll mark you before surgery.

You’ll have some devices placed on your legs called sequential hose that are stockings. And also we’ll have some devices called sequential compression devices, which squeeze your calves during surgery. And those will be continuously used throughout surgery and even in the recovery room until you go home. And then you’ll go home with the stockings.

A tummy tuck. We do all of these things because it has a higher risk of what’s called DVT, or deep venous thrombosis. And that in itself is a problem. But when that blood clot breaks free, it can go to your lungs or your pulmonary arteries and it’s called a pulmonary embolus. And that can be fatal. So one of the things we do for all our patients is we use those sequential devices and we also put them on a blood thinner. One is called Lovenox. It’s a shot you give once a day. The others called Xarelto, which is a pill you take once a day. And most of our patients take that for about seven days after surgery.

One of the things we like to do is evaluate our patients very quickly and every week. So we are going to see our patients every week for the first month after abdominoplasty. Because it really does have some significant downtime and patients have a lot of questions they need to have answered. And we want to be there for them every week to make sure they’re on the right track and everything’s moving in the right direction.
